INITIAL VISIT
A password from the office is required to open the initial forms, and will be given to you when you set up your first appointment.
Print all four forms if you are attending couple therapy; only the first three forms are needed if you are coming individually. (If two will be attending, both of you should read and sign these forms.**PLEASE READ THOROUGHLY ALL FORMS PRIOR TO SIGNING**)

You can also arrange to arrive 30-40 minutes early for your session, as an alternative, to complete the paperwork if you would like. All paperwork needs to be completed prior to being seen by the therapist.
